Few suggestions for rule tweaks to this item:

  • The magical effect should probably trigger a STR save rather than CON save (makes more sense based on how similar effects are treated)
  • The push mechanic is a bit weird, because it may put the target out of your reach denying you second/bonus attack. I'd say make it drop the target prone where they stand
  • Because of how the push works, you almost always want them to make the save and get extra damage instead. I'd tie the extra damage to bad roll on save
  • I'd say replace the 18-20 rule with once-per-turn, and drop the DC to compensate to streamline the effect.

Alternate suggested rule wording:

Once per turn, user may choose to perform a Whiplash Attack. If the target of size Medium or slammer is hit by the Whiplash Attack they take additional d4 force damage and must take a DC 10 STR save or become prone. If a creature that is size Large or bigger is hit by the Whiplash Attack, they take d6 force damage instead. Whiplash Attack emits a thunderous boom audible out to 300 feet.'

Notes:

  • The lower DC means more enemies will pass the save, removing the need for randomness.
  • Making it at-will attack rather than random gives reliable source of DPS that can be turned off
  • Lower damage output makes up for it triggering every turn
  • Making it force damage makes it less effective against creatures immune to magical damage
  • The sonic boom gives player reason not to use the attack (stealth, etc..)
